Many symptoms, especially headaches and migraine, tinnitus (singing in one's ears), attacks of vertigo, impaired vision or else speech problems (e.g. sibilants), as well as muscular tension in the neck and back area frequently have their origin in the region of the occlusal system.

If single or even several of the named symptoms occur, then there is a high probability that this is a case of so-called craniomandibular dysfunction (CMD). This is understood to mean disturbed function (dysfunction) in the interplay between cranium (skull) and mandible (lower jaw). In plain language: the upper and lower jaws are no longer in unison with one another, and as a consequence the force is irregularly distributed when biting.

The first indications of this functional disorder are, for example the proverbial gritting of one's teeth in stress situations, nocturnal grinding of the teeth or sensitive necks of the teeth. Another typical warning sign is the cracking of joints when opening or closing the mouth.

How does CMD arise? 

CMD is caused by abnormal positions of the teeth, tooth gaps, defective fillings, crowns or bridges that are not ground down exactly to bite height. Incorrectly fitted dental braces or prostheses also come into question as release mechanisms. Not least, incorrect posture, blockages of the vertebral column, high stress or other emotional pressure can play a major role.

If the dysfunction remains untreated, it can lead to permanent damage or arthrosis in the temporomandibular joints. For the body reacts with wear of the joint to mechanical stimulation and overloading of the masticatory muscles connected with this. Moreover, the tooth bed and the dental enamel are frequently damaged. Cracks or even deep furrows are caused in the occlusal surfaces due to the constant pressure, the gums can become inflamed and recede, and the teeth then frequently break apart.

It is therefore very important to track down and to remove disturbance fields in order to keep not only the occlusal system but the entire body healthy.
                
                                               
                    
 
What does functional analysis offer?

Precise values describing the position of the jaw in the skull, the movement of the temporomandibular joints and the position of the teeth in relation to one another are indispensable for the diagnosis and treatment of CMD and other orthodontic diseases. For only accurately acquired parameters enable an analytical evaluation of the situation of the mouth and jaw.

In this way, not only can the best possible preconditions for the intended treatment be created, but delayed damage to the temporomandibular joints can also be prevented.

Instrumental functional analysis is an extremely accurate measure if there is suspicion of functional diseases of the masticatory system. In this procedure the natural movements of the jaw in the masticatory processes are simulated by determining the jaw relation. Supplementary electronic jaw measurement (axiography) determines in addition the position of the upper jaw in relation to the skull and its distance from the temporomandibular joint. In a further step, a plaster cast of the upper and lower jaw is produced by reference to the evaluation, and mounted correspondingly in a special device (articulator), in order to perform an exact simulation of the masticatory processes for the restoration of the natural position between the teeth and the temporomandibular joints.



Therapy of functional disorders with DROS splints

If functional analysis finds that a dysfunction of the temporomandibular joint is the reason for the symptoms, the proven diagnostic-causal DROS-Therapy (Diagnostic Relaxation, Orientation and Stabilization) provides rapid relief in many cases. The stressed temporomandibular joint structures are relieved and the pains subside within a few days.

The most important components of this treatment method are light, two-phase DROS upper jaw splints fabricated individually for each patient from a high-quality plastic (cold polymerisates) in a certified dental technical laboratory.

The problematic effect of early contacts is rectified in the first relaxation phase and thus the stressed muscles are relieved. The second phase, the so-called orientation phase, guides the jaw back into its ideal physiological position. This is followed by stabilization of the temporomandibular joint system to maintain the achieved freedom from symptoms permanently.

Restoration of the tooth structure destroyed by CMD can commence after around seven weeks. A metal-free, high-quality restoration such as veneers, build-ups with enamel plus composite, ceramic or zirconium crowns, as well as implants if necessary is recommended to achieve the best possible functional and aesthetic result.


Are there other applications of functional analysis?
Apart from the above-mentioned symptoms in occlusal disorders and dysfunctions of the masticatory organ, its use has also been proven in restorative measures such as planning and fabrication of tooth build-ups, dental restoration, crowns, bridges or prostheses. It is also helpful in diagnosing periodontal diseases with faulty loading or extensive orthodontic treatment.
